OCR description
Conagra Brands Welfare Benefit Wrap Plan (Conagra), the covered entity (CE), reported that its business associate (BA), Automatic Data Processing Inc., inadvertently emailed documents containing the protected health information (PHI) of 1,713 individuals. The PHI involved included names, dates of birth, Social Security numbers, and health insurance information. Conagra notified HHS and affected individuals. Audit measures were implemented to mitigate potential data breaches.
