OCR description
The business associate (BA), Hawaii Pacific Health, reported that one of its affiliated covered entities’ workforce members impermissibly accessed the electronic protected health information (ePHI) of 3,772 individuals. The ePHI involved included names, addresses, phone numbers, email addresses, dates of birth, religious affiliation, race/ethnicity, Social Security numbers, health insurance information, and treatment information. The BA notified HHS, affected individuals, the media, and provided complimentary credit monitoring services to affected individuals. In its mitigation efforts, the BA sanctioned the workforce member. As a result of OCR’s investigation, the BA implemented improved technical safeguards to protect its ePHI.
