Golden Valley Health Centers

Golden Valley Health Centers, the covered entity, (CE), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 38,268 individuals. The ePHI involved included names, claims information, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff on recognizing and responding to fraudulent email communications. During the investigation, OCR provided the CE with technical assistance.
