OCR description
The covered entity (CE), Fackler Family Chiropractic, reported that it experienced a break-in at its facility and an iPad was stolen which contained the protected health information (PHI) of 1,731 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and local law enforcement. In response to the breach, the CE implemented additional physical safeguards and remotely disabled the stolen iPad.
