University of Minnesota Physicians, the covered entity (CE), reported that several employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 310,666 individuals. The PHI involved included names, addresses, telephone numbers, dates of birth, Social Security numbers, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In an effort to prevent a recurrence, the CE implemented additional administrative, technical, and security safeguards to better protect PHI. Staff were retrained on email security precautions.
