Washington University School of Medicine, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 14,795 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, diagnoses, lab result s and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. All staff were retrained on email security. OCR provided technical assistance regarding the HIPAA Rules.
