OCR description
The covered entity (CE), Child, Family and Community Services, Inc., reported that its facility was burglarized and multiple computers were stolen which contained the electronic protected health information (ePHI) of 1,658 individuals. The CE notified HHS, affected individuals, and the media. After OCR initiated its investigation, the CE ceased operations and OCR subsequently closed the case.
