Healthcare Resource Group, Inc., the business associate (BA), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 5,387 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, drivers’ license numbers, financial information, diagnoses, medications prescribed, and other treatment information. The BA notified HHS, affected individuals, and the media. As a result of OCR’s investigation, the BA implemented additional technical and security safeguards and retrained its staff. OCR received assurances that the BA implemented the corrective actions noted.
