OCR description
The covered entity (CE), Moses Taylor Hospital, reported that a tornado struck its medical records storage facility which was leased by its business associate (BA), STAT Informatic Solutions, LLC. This breach affected the protected health information (PHI) of approximately 1,905 individuals. The PHI exposed included diagnosis/conditions, lab results, medications, financial and other treatment information. The CE notified HHS, affected individuals, the media, and initiated a clean-up of the area to collect whatever records were recoverable.
