OCR description
The covered entity (CE), Poplar Bluff Regional Medical Center, reported that a tornado struck the Nashville, Tennessee area and damaged a building containing its medical records. The protected health information (PHI) involved included names, Social Security numbers, addresses, birthdates, medical record numbers, financial information, clinical information, treatment information, and medications prescribed. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also provided a telephone number for questions or concerns.
