Tennova Healthcare Harton

Tennova Healthcare – Harton, the covered entity (CE), reported that a tornado struck the medical records storage facility leased by its business associate (BA) which subsequently disclosed the protected health information (PHI) of 1,070 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, medical record numbers, financial information, diagnoses, medications prescribed, and other clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. Complimentary identity theft prevention services was also offered to all affected individuals. In its mitigation efforts, the BA securely destroyed the remaining records.
