OCR description
The covered entity (CE), Lisa Burkett, DDS, MS, reported that an employee sent a mass email to numerous individuals without masking the email addresses. The electronic protected health information (ePHI) involved included the names and email addresses for approximately 818 individuals. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE retrained its staff on the proper methods of securing ePHI via email. OCR obtained assurances that the CE implemented the corrective action noted.
