Alexander Chun, MD, PLLC

Dr. Alexander Chun, the covered entity (CE), reported that several boxes containing patient medical records were inadvertently left outside on a curb and later discovered by a television news station. This breach affected the protected health information (PHI) of 595 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and treatment information. Dr. Chun notified HHS, the affected individuals, and the media. In response to the breach, the CE strengthened his administrative safeguards and improved physical security. Staff were retrained on the requirement to protect and secure sensitive data.
