OCR description
The business associate (BA), PsyGenics, reported that an employee emailed the protected health information (PHI) of 614 individuals to his personal email account. The PHI involved included names, diagnoses/conditions, and other treatment information. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA sanctioned the responsible employee and implemented additional administrative safeguards to better protect its PHI.
