OCR description
The covered entity (CE), Renee Applebaum, PHD, reported that its business associate (BA) was the subject of a ransomware attack that affected the protected health information (PHI) of 3,800 individuals. The PHI involved included names, addresses, drivers’ license numbers, and Social Security numbers. The CE notified HHS; the BA notified affected individuals, and the media.
