OCR description
The covered entity (CE), Elite Emergency Physicians, (formerly Elkhart Emergency Physicians) reported that an employee of a vendor used by its business associate (BA) improperly discarded the protected health information (PHI) of approximately 550,000 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, health insurance and financial information, and diagnoses. The CE notified HHS and the media. In response to the breach, the CE destroyed the affected PHI and implemented new policies and procedures to better protect its PHI. The CE is no longer affiliated with the business associate or its subcontractor. OCR provided technical assistance to the CE regarding the HIPAA Rules.
