OCR description
The covered entity (CE), South Bend Medical Foundation, Inc., reported that its business associate (BA) improperly destroyed the protected health information (PHI) of approximately 500 individuals. The PHI involved included names, Social Security numbers, and lab results. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE properly destroyed the mishandled medical records and revised its policy and procedures to better protect its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
