Catholic Charities Diocese of San Diego, the covered entity (CE), reported that it was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of approximately 36,205 individuals. The ePHI involved included names, addresses, birthdates, drivers’ license numbers, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. Following OCR’s investigation, the CE revised its policy and procedures, retrained its workforce members, and implemented new technical safeguards.
