The covered entity (CE), University of Missouri Health Care, reported that employees were the victims of an email phishing attack that compromised the protected health information (PHI) of 5,074 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses/conditions, lab results, medications prescribed, health insurance and financial information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. A call center was established for questions or concerns. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its sensitive data.
