The Waldorf Center

The covered entity (CE), The Waldorf Center, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 608 individuals. The ePHI involved included names, Social Security numbers, drivers’ license numbers, passport numbers, financial information, and medical information. The CE notified HHS, affected individuals, the media, and offered complimentary credit monitoring and identity restoration services. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained its employees on recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
