OCR description
The covered entity (CE), The Kroger Company, reported that the protected health information (PHI) of 132,258 individuals was viewable via the Internet. The PHI involved included names, phone numbers, email addresses, birthdates, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect PHI.
