OCR description
The covered entity (CE), North Shore Pain Management, reported that it was the victim of a hacking attack that affected the electronic protected health information (ePHI) of 12,472 individuals. The ePHI involved included names, dates of birth, health insurance information, Social Security numbers, and clinical and financial information. The CE notified HHS, affected, and the media. In response to the breach, the CE hired a new IT vendor and implemented additional technical safeguards.
