OCR description
The covered entity (CE), CodeMetro, Inc., reported that it experienced a cyber-attack that affected the electronic protected health information (ePHI) of approximately 62,807 individuals. The ePHI involved included names, Social Security numbers, addresses, dates of birth, diagnoses/conditions, health insurance information, and claims information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following OCR’s investigation, the CE retrained its workforce members and implemented additional technical safeguards.
