Friendship Community Care, Inc.

Friendship Community Care, Inc., the covered entity (CE), reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 10,845 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license information, Social Security numbers, diagnoses, health insurance information, facial photographs, and medications prescribed. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards and retrained its workforce to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
