Grace & Porta Benefits, inc.

The covered entity (CE), Grace & Porta Benefits, Inc., reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 572 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, financial information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff on email security.
