OCR description
WellCare Health Plans, Inc., the covered entity (CE), reported that its business associate (BA) released documents containing the protected health information (PHI) of 2,794 individuals to a court. The PHI involved included names and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA provided additional HIPAA training to employees regarding the need to safeguard PHI. The CE provided affected individuals with complimentary credit monitoring services. OCR obtained assurances that the CE and BA implemented the corrective actions noted.
