OCR description
The covered entity (CE), Florida Orthopaedic Institute reported that a ransomware attack affected the protected health information (PHI) of 651,527 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, claims information, diagnoses, medical conditions, laboratory results, and medications. The CE notified HHS, the affected individuals, the media, and provided substitute notice on its website. Following the breach, the CE implemented additional technical safeguards, conducted employee training, and offered identity restoration services to individuals.
