OCR description
The covered entity (CE), Four Winds Behavioral Health, reported that employees were the victims of an email phishing attack that affected the protected health information (PHI) of 500 individuals. The PHI involved included names, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on email security.
