OCR description
Accu Copy of Greenville, Inc., the business associate (BA), reported that it was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 21,800 individuals. The ePHI involved included names, dates of birth, addresses, and clinical and treatment information. The BA notified HHS, affected individuals, and the media. In response, the BA implemented additional administrative, technical, and security safeguards and retrained employees on safeguarding its ePHI.
