OCR description
The covered entity (CE), Tak-ming Ko, M.D., LLC, reported that its former business associate experienced a ransomware attack that affected the electronic protected health information (ePHI) of 1,158 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims information, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. The CE also provided complimentary credit monitoring to affected individuals.
