OCR description
Greenway Health, LLC, a business associate (BA), reported that it experienced a cyber-attack that affected the electronic protected health information (ePHI) of 2,490 individuals. The ePHI involved included names, Social Security numbers, and clinical and health insurance information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA reviewed and revised its security policies and strengthened its technical safeguards.
