OCR description
US HealthCenter, the covered entity (CE), reported that an employee of its business associate (BA) was the subject of an email phishing scheme that compromised the protected health information (PHI) of 1,441 individuals. The PHI involved included names, birthdates, addresses, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative and technical safeguards to better protect PHI.
