OCR description
The covered entity (CE), Maryland Health Enterprises dba Lorien Health Services, reported that it experienced a cyber-attack that affected the electronic protected health information (ePHI) of 537 individuals. The ePHI involved included names, dates of birth, and medical and diagnostic information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE strengthened its technical and security safeguards to better protect its ePHI.
