OCR description
The covered entity (CE), SuperValu Pharmacies, Inc., reported that six of its pharmacies were burglarized affecting the protected health information (PHI) of approximately 1,593 individuals. The PHI involved included names, addresses, dates of birth, financial information, medication information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional physical and procedural safeguards. OCR provided technical assistance to the CE regarding the HIPAA Breach Notification Rule.
