OCR description
Aetna ACE, the covered entity (CE), reported that employees of its business associate were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 1,084 individuals. The ePHI involved included names, addresses, telephone numbers, diagnoses, health insurance information, and treatment information. Aetna notified OCR and affected individuals.
