Dynasplint Systems, Inc.

Dynasplint Systems, the covered entity (CE), reported that multiple employees were the victims of an email phishing attack that affected the protected health information (PHI) of 102,800 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, lab results, medications prescribed, and financial and other treatment information. The CE notified HHS, affected individual, the media, and provided complimentary credit monitoring services. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI.
