OCR description
The covered entity (CE), The Iowa Clinic, reported that an employee inadvertently mailed the protected health information (PHI) of 915 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE sanctioned and retrained the responsible employee and revised its procedures to better protect its sensitive data.
