OCR description
Idaho State University Dental Hygiene, the covered entity (CE), reported that an employee inadvertently sent an email containing the email addresses of 543 individuals that were unmasked. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE strengthened its administrative safeguards and retrained its staff. During the investigation, OCR provided the CE with technical assistance.
