OCR description
The covered entity (CE), Advocate Aurora Health, reported that an employee mailed documents containing the protected health information of 1,907 individuals to the wrong recipients. The PHI involved included names, addresses, health insurance and claims information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative safeguards to better protect its PHI.
