Hamilton Health Center, Inc.

The covered entity (CE), Hamilton Health Center, Inc., reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 10,393 individuals. The ePHI involved included names, health insurance information, dates of birth, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and provided additional training on the proper methods of recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
