HealthFirst Bluegrass

The covered entity (CE), HealthFirst Bluegrass, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 971 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims information, financial information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also offered complimentary credit monitoring services to all affected individuals. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative and technical safeguards. OCR obtained assurances that the CE implemented the corrective actions noted.
