The covered entity (CE), City of Lafayette, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 9,149 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license numbers, Social Security numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its ePHI. OCR provided the CE with technical assistance regarding its HIPAA Security Rule obligations.
