OCR description
The covered entity (CE), Erlanger Health System, reported that its business associate (BA) lost CDs that contained the electronic protected health information (ePHI) of 4,938 individuals. The ePHI involved included names, dates of birth, and lab results. The CDs were not encrypted or password protected. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Following the breach, the CE trained the staff of its BA on the proper methods of safeguarding ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
