OCR description
The covered entity (CE), Carnegie Tri-County Municipal Hospital, reported that it was notified that documents containing the protected health information (PHI) of 1,076 individuals was found in an unsecured location. The PHI involved included names, dates of birth, medical record numbers, Social Security numbers, and financial information. The CE notified HHS, affected individuals, the media, and local law enforcement. In its mitigation efforts, the CE retrained its workforce members on the proper storage and disposal of PHI.
