Starling Physicians, PC

The covered entity (CE), Starling Physicians, P.C., reported that two employees were victims of an email phishing attack that affected the electronic protected health information (ePHI) of 7,777 individuals. The ePHI involved included names, dates of birth, Social Security numbers, addresses, and health insurance and medical treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In addition, the CE offered complimentary credit monitoring services to affected individuals and established a toll-free phone number for questions or concerns. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
