OCR description
The covered entity (CE), George West Mental Health Foundation dba Skyland Trail, reported that its business associate experienced a ransomware attack affecting the electronic protected health information (ePHI) of approximately 2,724 patients. The ePHI involved included names, addresses, phone numbers, email addresses, gender, and health insurance and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
