OCR description
The covered entity (CE), Our Lady of the Lake, reported that its business associate experienced a ransomware attack affecting the electronic protected health information (ePHI) of approximately 31,166 individuals. The ePHI involved included names, mailing addresses, dates of birth, email addresses, telephone numbers, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
