Community Surgical Supply of Toms River, Inc.

The covered entity (CE), Community Surgical Supply of Toms River, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 31,550 individuals. The ePHI involved included names, birthdates, addresses, drivers’ license numbers, Social Security numbers, diagnoses, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its technical safeguards and retrained its staff on the proper methods of identifying fraudulent email communications.
