Pollack, Barbaccia, & Guzman MD PC

The covered entity (CE), Pollack, Barbaccia, & Guzman, MD, reported that its storage unit was burglarized and several boxes of medical records were stolen. This incident affected the protected health information (PHI) of 1,135 individuals and included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, financial information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, security, and physical safeguards to better protect its PHI.
