Alliance Spine and Pain Management, PLLC d/b/a Freedom Spine & Pain Center

Alliance Spine and Pain Management, PLLC, dba Freedom Spine and Pain Center, the covered entity (CE), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 2,235 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, drivers’ license numbers, and clinical and treatment information. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. Additionally, the CE retrained its workforce members on recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
